H1N1 Consent Forms

Attention Parents/Guardians of USD 388 students,

The H1N1 Vaccine Consent Form has been sent home with all students this week. If you haven’t seen it, please check with your child. Extra forms are at the grade school and high school office. The director of the Ellis County Health Department advises that you should not assume immunity from H1N1 unless you have actually tested positive for the virus!  We have seen an increase in absenteeism at the grade school this week with a variety of symptoms, including fever, stomach aches, vomiting, a few coughing, and strep throat at the 3rd grade level. Some children seem to be better then spike temps again.  Please keep your children home till they are symptom free 24 hours without the aid of Tylenol/Ibuprofen. Printable Consent Form
